HIV/AIDS Case Management

Case Managers can assist you with your health care needs, as well as linkage to care. The coordination and follow-up of medical treatments is another important role in medical care management.

HIV Nurse Case Managers will help you navigate the health care system. They do your initial assessment and enroll you in our Ryan White Program. Case managers maximize your provider visits; they get your laboratory work in order and schedule your appointments. They want to know how you are feeling and how you are responding to treatments and medications. Nurse Case Managers can explain laboratory tests and help you understand HIV disease and medications. They provide medication education and adherence support as well as coordinate your health care. They are here to help solve problems about your illness, medications, financial issues around health care, and substance abuse issues. HIV Case Managers are a resource for YOU.
