How to Find Your Activation Code
You will need your Activation Code to access MyChart.
Here are the ways to get your activation code:
MCHC includes your Activation Code on your After Visit Summary and also sends it by text or email. If you have your Activation Code, visit MyChart and follow the instructions to sign up or access your account.
