Enroll and take advantage of a new feature “PreCheck-In”.
No need to wait in a long line and complete most check-in tasks from the comfort of your own home before you arrive to our clinics! You can update documents, questionnaires, medications, allergies, and much more! This will be available for all patients with or without MyChart.
PreCheck-in will be Introduced at Different Dates by Location
July 15th – Dora St. Clinic
August 25th – Hillside Clinic
September 15th – Lakeview Clinic
October 6th – Little Lake Clinic
How to Use PreCheck-In:
If You Already Have MyChart
If you have MyChart, login to MyChart and the PreCheck-In button next to the appointment will be available 72 hours prior to your appointment time. Click the PreCheck-In and follow the prompts and it will guide you through each section that needs to be completed.
If You Do Not Have MyChart
If you do not have MyChart: you will receive an email to the email address listed on your Epic account with a link 72 hours prior to your appointment time that will bring you to the PreCheck-In activity. Follow the prompts and it will guide you through each section that needs to be completed.
