Sign up for your MyChart account:

You can sign up for a MyChart account by clicking here  or in the MyChart mobile app.

IF YOU DON’T HAVE AN ACTIVATION CODE

• Desktop: Visit our website (www.mchcinc.org). click the MyChart login logo in the header, then click Sign Up to request an activation code.
  • Follow the steps to verify your identity and create your MyChart account.
• Mobile Devices: Download the MyChart app. Click “Add Organization. Search for “MCHC.” Then click “Sign up.”
  • Follow the steps to verify your identity and create your MyChart account.

USE YOUR ACTIVATION CODE TO SIGN UP

• Look for an activation code on your After Visit Summary or in an email or text message from your healthcare organization.
• Follow the instructions provided with your activation code to create your MyChart account.

ACCESS TO YOUR MEDICAL RECORDS ONLINE, 24/7, SAFELY AND SECURELY

• Read the medical information your Care Team has written
• Keep track of your medications, lab results, immunizations and more
• Learn about your medical conditions from a trusted source (HealthWise)
• Access tips for staying healthy
• Get a summary of care from your visits to share with specialists outside your Care Team

RECEIVE HEALTH REMINDERS

Health Screening reminders are noted in MyChart. You may also receive reminders from your Care Team.

MANAGE YOUR APPOINTMENTS

• Request or cancel an appointment
• View upcoming and past appointments
